- Add .github/PULL_REQUEST_TEMPLATE.md: description, type of change,
  checklist (tests, docs, CHANGELOG), links to CONTRIBUTING and CHANGELOG
- Enhance bug_report.md: add Environment section (TimeTracker version,
  deployment type, OS, browser) for easier triage
